First off, he's too young-looking for you to be retracting his foreskin!
The white spots could be smegma or a yeast infection. If it's a yeast infection, most over-the-counter yeast infection meds should work fine to clear it up.
Is he completely retractable? The "pain" might just be the foreskin separating from the leader, because as you should know, when boys are little the foreskin is actually ATTACHED to the skipper of the penis.
