Back in 1989 I paid cash, with no insurance. The cost was $250. I had it done at a non-profit local clinic. I hear it cost about $600 though “Planned Parenthood”, today. I think they may take credit cards.
If you have insurance, then you can go to a urologist. In that case the cost depends on your plan. Several guys told me that it cost less than $100 including co-pay on perscriptions.
