You sound just like me. I know this sounds weird, but you might have ADD. A common misconception about ADD is that it means you cant pay attention, but what it really is is the lack of being able to choose what to pay attention to. You should see a doctor, and if you really have it than take a medication.

You may have ADD or ADHD, either way you should tell someone and you should be tested for both. If you can not concentrate or sit still it may be ADHD. If you simply can not concentrate it may be ADD.
I've suffered for anxiety, and know the feeling. You can't pay attention, and you almost want to cry when you try to confront stressful situations. You should talk to your doctor about anxiety and depressive orders. Prescribtions such as Zoloft and Paxil can help you gain control.
It's called procrastination. I do it, too. However, you have to tell yourself to focus and just do it! Once you get started on the homework it won't be so bad to finish it -- it is just getting started that is the hard part.
