Why do i lose my train of thought when I talk?

I often find that when i am answering a Question at work or in a meeting i suddenly lose my train of thought and start rambling in all directions. how do i fix this? Are there any exercises? any Tips will help.

For your own peace of mind, I would go talk to a mental health specialist just to be sure I was doing okay.
