There was this study done in the southeast somewhere and in northern Europe wondering how and why the atkin's diet works. To make a long story short, they compared 4 diets.., slim fast, atkins, counting calories and jenny Craig (or something like that). The people on the atkins could eat what ever they wanted except certain carbs while everyone else limited their food intake. They had everyone takes notes on what they ate. Even thou the atkins people could eat what ever they wanted, they consumed the least calories. What they found out is when someone eats a high amount of protein, that the brain releases a chemical which makes them stop eating. It isn't that people are counting their carbs but eating high amounts of protein that allows them to drop the weight. So what I've done it make sure I include a healthy portions of protein along with food from all the food groups when I eat (including bread etc..). Also no sweets. This is the best way that I know of to stop yo-yoing because you don't crave foods when you eat like this. This is from my personal experience anyways.

You need to find a fun workout that you regularly will enjoy doing and that won't get boring after a couple weeks. Keep doing this workout, along with eating right, and you should stay in shape.
I think it's a matter of not falling back into old lifestyle habits, but that is so difficult.

Diet is not a temporary thing, it's a lifestyle change. If you really must eat "the old way" get more exercise. The key is to spend more calories than you consume. If you take in the same amout as you spend, you keep your weight steady.
"Diet" is a four letter word. It's a bad word. It causes yo-yoing. People who diet to lose weight usually gain it all back, and sometimes even more. The best way is to lose only 1 to 2 lbs a week. And try to lose the body fat instead of losing the weight. It's much healthier this way.
What you need is a lifestyle change. Eat healthy. Eliminate the junk food and fast food. If you get the munchies, try eating carrots instead of chips. Spread out your meals and try to eat 5-6 smaller meals a day. Check the labels when you shop.
And, find a good exercise program. You want something that you will enjoy to keep you coming back for more. Weight training is a very good program to keep the weight off. It builds muscles which will burn more calories at rest. And the more calories you burn, the less fat the gets deposited on your body.
