I have the same problem. I tend to focus on background noises instead of people's voices and it drives me crazy, too. I went to the audiologist and they ran some hearing tests and said I was fine (even though everything had to be repeated before I knew what he was saying, which in my opinion is NOT fine). You're mind probably is constantly on the go, like mine, and you can't decipher you're thoughts from other people's words. I hate talking on the phone because of it.
I have started learning sign language and most of my friends now are Deaf so I don't worry about it as much (though it still takes a while for things to register). Of course that is kinda drastic.
Try listening to music and try to pay attention to the lyrics. Try Broadway music where the songs are conversational. You can try and train your ear to hear and understand the words. Or watch Jeapordy or a show like that where you can see and hear the clues and then have to answer and comprehend.

It could be your hearing itself, but it sounds to me like you're not comprehending (processing) what they are saying. Do you give the speaker your full attention when they speak to you? Some people process slower than others, but it may not hurt to talk to your doctor who can refer you to a speech therapist that can help.

I assume when you write this that you are a native English speaker and that your concerns aren't stemming from learning English as a second language. If not, then it sounds like you might want to first make an appointment with a qualified audiologist. When you meet with them and report your concerns, they will likely do a Central Auditory Processing Disorders (CAPD) test on you to see if you have difficulty with processing verbal information. Depending on the results, they may recommend several compensatory strategies, however it is unlikely that you will find a "cure" for this. How old you are and how badly it's affecting your ability to work and socialize are things to consider. In other words, how handicapping is this to your daily functioning? I hope this helped.
