Does an annular tear ever heal?

Of course. Unless the tear is complete. Most annular tears are relatively mild but create a significant amout of pain and inflammation. For most people, if you can stick out the pain and do not aggravate the tear, the body has the natural ability to smooth out tthe rough spots and fill in the tear. Over time it will "heal". As the pain diminishes, you will need to rehabilitate the knee, as all of the pain and guarding will cause a degree of atrophy. During this phase, loading of the joint will continue the re-modeling process.
With time and luck surgery may be avoided.
(Significant tears that require excessive healing, will most likely NOT respond well to conservative treatment, consider surgery if the pain is still significant for 1 - 2 months post injry)
