question reguarding sprain?

okay. so i sprained my foot/ankle 8 months ago. the ankle healed. the foot did not. now my dr. says i need cortizone and a cast. if that doesn't work. surgery i'm guessing. any opinions on this? has anyone experienced this or can offer some help? would i be in a cast after the surgery too?
