Well medical personnel will always say you should never put anything smaller than your elbow in your ears, and since it's impossible to put your elbow in there basically you shouldn't use anything- BUT. I had a swab taken from my ear a few months ago, and that was like a cotton bud so so much for their advice!

You are not supposed to put anything in your ears. By using Q Tips you are actually compounding the wax in your ears, not a good thing to do.

Ears are actually self-cleaning. Ear wax (cerumen) is like the mucus in our nose - is there to trap dirt, debris and insects from damaging the eardrum. There are hair cells (cilia) to move the wax toward the opening of the ear canal, so it can be washed away with your washcloth and fingertip.
If you put Q-tip/swabs in your ears, not only could you injure your ear or pack the wax deeper against the ear canal, you also damage the cilia, so your ear can no longer clean itself efficiently.
Some people are prone to wax build-up, either due to having a very small ear canal or dry wax, etc. They can help the wax stay soft by using over-the-counter drops.
Of course it's best to get the ears checked and cleaned (if necessary) by a physician, and to follow the doctor's recommendations for cleaning.
