Yes, it's weird. My grandfather smoked until the day he dies which was 86 years old and my friends grandfather who is now 97. still smokes.
Other Answers:
Genetics, Diet, How much oxygen you breathe, how much water you take in.. There are a lot of factors that come into someone's health. Your grandfather is blessed with good health. It's good luck. Be Very happy.
I guess this goes to show doctors don't know everything. when its your time to die you will, God has a reason for all things :)

He has a very strong, perhaps unusually strong, physiology that enables his body to resist celular mutations.
what is your Grandad eating because i want some of it!! I have no idea how this has turned out like this for him but i congratulate him and ask if he also has nothing to eat but very healthy food and has a very vigorous exercise program..could help.
That is wierd, but hey, I know people in their 80's that have smoked almost all their life, and they have no cancers and they're perfectly healthy (aside from the old-people's diseases). Makes me more comfortable that I'm a smoker man. WHOOOT! POWER TO THE CANCER STICKS!
well, i used to know a guy who swore it was the filters that caused cancer and not the cigarettes. his grandpa smoked unfiltered lucky strikes until he was 90 and died of a heart attack.
maybe your grandfater starting smoking at such a young age caused him to develop some weird immunity to most of the hazards or something odd like that.
it depends on the person.
It's quite strange but people from the olden days are (were)much healthier than people in today's times. Probably cause of the stress levels and lifestyles
How often did he smoke?
I don't smoke because every person in my family that has died has died from some type of cancer caused from smoking. My grandpa right now has to have a breathing machine on at his house 24/7 and when he leaves the house he has a portable one not to mention all the other things wrong with him because of him smoking for 40 years. Your grandfather is probably one of the lucky ones.
Doctors are not always right.
Genetics play a huge part in how we respond to "bad things"
How much does he smoke? What's his diet? Does he exercise? Does he drink tons of water? Does he take vitamins? Does he laugh a lot? Is he surrounded by family that's close by? These are just a few of the factors that can affect people's lives. Some people are prone to diseases more than others.
My Dad started smoking at the age of 13, He lived to be 81. He did die of Lung Cancer, very rapidly. He was diagnosed with a Tumor, (stage 1) 4 Weeks later he was dead. My personal opinion is the tobacco that my father used, and probably your G-Father is different than what is used today. The tobacco institutes use more chemicals today that they did in previous years. I think its the process and chemicals they use that makes it so dangerous.
