I don't know the full scope of your back condition. But I would recommend a non-medication approach. Try Chiropractic or Acupuncture. Both are great a relieving even long-standing back pain (depending on what is wrong with you, of course).

If the specialists are physicans they should be able to prescribe a painkiller for you regardless of what your primary care provider says. Or am I just not understanding your question?
Now, don't throw something at me. Try to see this from your physican's point of view. Is it possible the pain meds have done all they can do for you, especially after 5 months, and you're now addicted to them? Could that be why you're in pain?
Have you looked at non-medical ways to deal with your pain? There are a lot of options out there.
