Well, the outcome of death is pretty extreme. Long-term GERD may predispose you to something called Barrett's esophagus, which is a change in the lining of your esophagus. This, in turn may (as in not always) lead to esophageal cancer. If your GERD is extremely bad and you have to have surgery, when you are induced for anesthesia (getting put under general anesthesia) and have an episode of reflux, this may go down your lungs. When this happens, it leads to an aspiration pneumonitis. This can be extremely detrimental and lead to pneumonia, sepsis, and eventually death. I would argue that GERD and stomach ulcers are two separate disease processes (but often linked). Stomach ulcers can be treated with medications including those used to GERD, but a bacteria called Helicobacter pylori is often a source for stomach ulcers. GERD can be caused by an incompetent esophageal sphincter like a hiatal hernia.

Yes it can, but that's really extreme. What happens is that the acid eats away at the esophogus and causes a bleeding ulcer. Then you bleed to death.
