Never heard of it, but based on a general understanding of the English language, it is a shortage of Alpha 1 Antitrypsin.

alpha one antitrypsin is an enzyme present in our lungs
it is required for the elastase - anti elastase balance. i.e
smoking destroys it.
and our lungs permenantly dilate as a result. this leads to functional insufficency..
