People say they do because they don't want to be seen as irresponsible. I think half the time it "breaks" people are lying. Many people also claim they get pregnant and they always used a condom properly, again, I have trouble believing it, but OK.

Anyway yes there are ways for condoms to fail:

1) Condom has hole in it.
2) No air at the top and it seeps out the bottom
3) Contact during pre-game activities
4) Condom has hole in it.
5) Condom was put on inside out, then put on the other way when the guy realized that it was wrong.
6) Condom breaks
7) Oil based lube like vasalene or massage oil eats through it.
8) Condom pops off inside (due to lubrication inside, don't do that)
