what causes 49 y/o has spell at 11am-2pm, cant focus eyes,lethargic,. pain blood sugar ok happens every 3-4da

pt. is bi- polar, on respiradal& tegretol 2x da. spell always comes on between 11am & 2pm. usually 3-4da apart. is non-insulin diabetic, has pacemaker which was checked & is ok. when he has spell he cant focus eyes,think straight, hard to function, [wont go to Dr.!] Could this be his meds, or something more serious, possibly TIA's why only at this time of day? never has been any other time.Says "he feels like he is druged up" Have taken his vitals during these spells, and all normal,he is a little pale at these times, speach slows, he is also over weight, high colesterol. he takes meds at bedtime & breakfast time, also takes 81mg.asprin daily. works construction, has to almost stop when this happens, slows down when driving, this is always at the same time of day, never any other time, whats going on with him?
