You need to re train yourself to stand or sit straight. Sometimes when you are at home you can wear a Velcro strap around your waste and work around the house with it. It will remind you to stand straight. But you will need plenty of exercises and control.

you've trained your muscles to slouch, now you have to train them to sit straight. so try to keep reminding yourself about your position and eventually you won't have to think about it any more.
Get a book on Yoga and start with the basics.this really improves your posture not to mention you feel better as well.
