Can anyone with Hypoglycemia recommend a reliable way to lose weight when diet and exercise fail?

Honestly, its better you consult a specialist to find out what's best for you. I really want to help but I don't think I can give better answers than specialist do. Sorry.
good luck
Love is omnipresence, Love is patient, love is kind. It does not envy, it does not boast, it is not proud. It is not rude, it is not self-seeking, it is not easily angered, it keeps no record of wrongs. Love does not delight in evil but rejoices with the truth. It always protects, always trusts, always hopes, always perseveres. Love never fails.

The reason they fail is because most people tell you to do low fat and cardio which does not work very well. Low carb and weight lifting is the way to go. High intensity interval training is also very effective. Do a search on it.
Bottom line is, you need to include exercise into a weight loss program/life style change.
Having said that, if you are having low blood sugar after exercising, you need to adjust what you eat before you exercise and the intensity of the exercise.
It also helps to eat some protein with your carbs. For instance, rather than eating an apple, eat an apple with a bit of peanut butter.
By all means, check in with your doctor. If you've been exercising and watching what you eat and still haven't seen any results, your doctor needs to know.

There is not easy way to loosed weight without diet and exercise. You have to change your diet by lowering your calorie intake and eating more fruits and vegetables. Drink plenty of water and exercise. Cardio is the best! When your heart is at an elevated heart rate for 15 min that is good cardio, but anything after 15 min at an elevated heart rate is weight lose. You won't see changes overnight because that is not how long it took you to put the weight on. Stick with it and don't get discouraged! Good Luck!!
Make sure you do weight bearing exercises.
Make sure to avoid low fat diets.
Stay away from proccessed foods - and artificial sweetners!
Eat healthy fats (i.e. avacados, olive oil, sunflower oil, etc).
Check out the dangers of bad oils/fats:

Avoid unhealthy fats (i.e. canola, partially hydrogenated -- read the food packets!) Some sites ignorantly recommend Canola as a healthy fat/oil which it is NOT. Check out the dangers of Canola: http://www.nexusmagazine.com/articles/canola.html

Pretty simple rule:
If it's been tampered with to alter it's natural properties - don't eat it.
A combination of a load bearing exercise regime with a healthy higher protein diet which excludes processed, refined carbs produces the healthiest results when trying to regain fitness and lose excess weight. Note: I say highER protein diet - not high - because it's important to have a balance of good carbs with healthy proteins. Check out the LOW GI foods link above -- it's really helpful in maintaining blood sugar.
Im hypoglycemic too, with the same problem, last week I found this forum;

basically a woman in it says she uses the Zone Diet to maintain her sugar levels, and keep her weight down. Im going to follow this diet as well

This is what she says"""
Hi,
The Zone Diet was created by Barry Sears PhD, and has been around for the last 10 years. I have been following it for 9 years and find it fantastic. (You will find he has a number of books and a website www.drsears.com)
I suffered a number of things, reactive hypoglycemia, bad PMT, severe menstrual pain, swollen knees, stiff neck and TMJ. (I have tested positive to Lupus at one point, so it could be a mild version.)
The zone diet is balanced, protein is eaten at every meal ( as small p0rtion eg 3 oz chicken breast). Carbohydrates are restricted and the type is important. The idea is to get a low glycemic load from each meal so that blood sugar never peaks and stays even for the next 4 hours. Glycemic index is important, the food that works best for me have a low GI (below 50, preferably below 40) That means I choose grains like whole grain oats (50) and barley (25). Veggies - fibrous mostly above ground veges as green beans, broccoli, courgettes etc, which are low density (dont break down into much sugar) and very low GI. I would have big plate of those at a meal, often as a salad. Having lemon juice or vinegar on a salad with a meal slows down the rate at which carbs digest into sugar so that has an added bonus. I only have very small amounts of starchy veggies as they are very dense. A cup of cooked rice for example will convert to 45 grams of glucose when digested, that's 10 teaspoons of sugar. So even if it has a low GI (eg basmati about 55) the amount of total glucose released from that meal will be too high for my body to manage.
Glycemic load = grams of carbs x Glycemic index
then divide the answer by 100.
This gives the actual sugar load your body has to cope with.
So a cup of rice gives a sugar load of 25 or more
Whereas a cup of cooked porrige gives a load of 13.5 - a big difference!
A cup of green beans has a GL of only 1 by comparison.
To each meal I add a small amoount of good fat like olive oil or nuts, fats help slow digestion of carbs and release of sugar into your bloodstream, and eating good fats is the better choice healthwise.
I snack 2 - 3 times a day between meals, usually a small piece of low GI fruit like a plum and a matchbox size of low fat cheese.
The results of eating this way was dramatic, I lost weight easily without getting hungry or dizzy. My energy levels improved, I slept better, I felt clearer in my head and more positive. My PMT completely disappeared - and has stayed away.
About 8 years ago I added a hefty dose of Omega 3 fish oil with a dramatic improvement in my joint problems and vitual elimination of Menstrual pain. I also take other supplements which all add to the improvements - a good quality multi with high mineral and B content plus extra E and C antioxidants.
I can truely say I have never felt more well than I am now, at age 45.
