There is a condition connected with diabetes that causes one's breath to smell sweet (it actually smells kind of like nail polish remover). It is called ketoacidosis. This is very serious. this occurs when you have WAY too much sugar in your blood, so much so that your body is trying to get rid of it any way it can, including breathing it out. I don't know about the tongue coated in sugar, but that is all I could think of
