In rare cases (2-5%) surgery may be required when skin is broken or if the fracture is extremely displaced (disunion). If it is green stick fracture no treatment is necessary except collar and cuff. If it is middle third fracture with displacement, Reduce the fragments by pulling the patients shoulders firmly, backwards without anesthesia. So to keep the displaced fragments in place for alignment the following immobilization methods are followed after reduction. a) Using conventional felt rings, b) Figure of eight bandage, c)
Clavicular rings. d) Clavicular Cross.
Immobilisation is continued till the tenderness at the fracture site persists or for 3 weeks. If the fracture is at the lateral end and if thee is no displacement apply collar and cuff for 3 weeks. If there is displacement apply adhesive plaster round the shoulder down the arm, below the elbow , up the arm and back to the shoulder. Use in addition collar and cuff. Nutritious diet , milk and calcium tablets for three weeks.
