Well I am not 100% positive here, BUT I would say it is because with the drugs they give you and or putting you to sleep relaxes you completely. This being, if you ha d a full bowel then there is a relatively a good chance that you will have a bowel movement (take a dump) while you are in surgery. YES this could be messy as well as smelly.
OR it could be as stated above.

Because if you throw up from the anesthesia, you will be unconscious and may not get it out of your mouth. you could end up breathing it into your lungs and getting aspiration pneumonia (pneumonia from breathing something in). This can be dangerous or fatal. It is actually very important not to eat or drink before surgery. If you are dying of thirst, rinse your mouth and spit, but don't swallow.

There are multiple reasons depending on the surgery, but the main reason is that the anesthesia given during many surgical procedures can cause vomiting. This can be particularly dangerous and can lead to serious complications.
