Improper collection or inadequate specimen; specimen more than seven days old in liquid-based preservative; specimen without identification; specimen that has leaked in transit and does not have the required volume; Gen-Probe® Aptima® collection tube with multiple swabs; unlabeled specimen or name discrepancy between the specimen label and the request form.
