I have no idea of the cost but if you are considering getting it, be very sure you need it, because joints and especially that one are very slow to heal - I've had symptoms of a tear in that joint for months, and the night-time twinges are finally fading. You will need gentle exercise for your shoulder under the advice of a physiotherapist whether or not you have surgery.
If you really do need it, then your insurance should cover it!
