i would like someone working in a care career to answer this too if possible

Answers:
people don't always say what they mean, and often cannot communicate with strangers. for example, they may say "it doesn't hurt" but the look on their face or the way they move may tell you differently. as a nurse, you encounter many situations where we have to rely on verbal and non-verbal communication. they may even tell you they are in extreme pain, but your observations may tell you they are moving around fine and simply seeking medications. others may not want to tell you everything, like in mental health, they may say everything at home is fine, when in reality, the bruises may tell you everything is not fine. there are an infinite amount of examples i could give you, i hope these help.
not just for care practitioners, but i know in the world, the best way to communicate is through human touch and emotion. words can be repeated over and over again and mean nothing. i am not sure if this was the way you were asking but that was my interpretation of the question.
