Help please ,where can i apply on line to obtain the replacement e111 form?

You can do it on the web: can't remember the address though, am sure it would come up on a google search

If were talking Uk, The e111 form is no longer used it is now the EHIC (European Health insurance card) I don't know if you can apply online although I should think that you would be able to. Try typing EHIC in the search engine.

Its free and on the link below, it is the official UK Government website, the card takes a few minutes to apply for and lasts several years now

I found that it was much quicker on the phone, they say 3 weeks online, but the cards came in 48 hours by phone.
