My child is going on vacation with grandmother how do I designate her legal for medical purposes?

How do I designate her as his caregiver for the week when we are not going to be there in case of an accident? So she can use our insurance

It sounds like you are talking about an Emergency Authorization. It would be similar to what you sign for daycare. A form stating that you give grandma permission to seek medical treatment for your child.

Simply write out your authorization or type it out on the computer, print it for the grandmother to have in her possession. List your contact numbers, dr. numbers, you might even copy your medical ins. card or give her yours. In cases of emergency with minors you really only need to provide I.D. as gaurdian.
