I'd take a wild stab at it and say,,, you can't. If you do, it is illegal and you and the person that forged it are both liable for criminal fraud which can land you in jail for a very long time.

That's like asking how you buy bread without going to a place to buy it. Go to the doctor. Or you could forge it. You are on a computer, surely you have a basic word processor at least. But that's illegal. You shouldn't do illegal things.

Depends what it would be for, I would think, but I have done that, and it is legal since you are getting it from them.
Use word to make a fake dr appoint slip then write the signature so no one can read it, or find an old one and make a photo copy and change the date/time.
you can't because the doctor needs to assess/see you first and so he can find out what is wrong with you.
Ask yourself if you have any friends who work in doctors offices? If so, see if they can get you one. Recently, this worked for a friend of mine who took a 'sick day' without going to a doctor.

If you have a regular doctor who you have been seeing and it is just a routine note for school or something call his nurse and ask her to get the doc to sign what ever she writes up for you and then ask him/her to fax it to whoever needs it. Most docs will do that for a regular patient.

Technically, can't be done and legal. However, that said, you can copy one and forge your name and hope it's good enough to pass review. Years ago when computers were coming out and laser printers were new, it was possible to fool people into thinking such notes were real. But today, with laser printers all over, you would have a problem making it even seem real. It would be easier to just go to the doctor's office and get one. Then you can try and change it. At least, you have an original to work with.
