its actually really hard to tell from what is explained here , noone can really be sure unless you get an xray and an ekg , etc. , best thing to do is go to the doctors to get it looked at , but from what i get from this im thinking its a pinched nerve in your back , but since you didnt say what kind of work you do , gender , age , etc. , impossible to diagnose over the net.

I think you grandpa needs to get a general check up which includes x-rays and ultrasounds of all the back and abdominal area to find out exactly in which part of the body the problem is in so it can be treated appropriately.
