Why is it that condoms and birth control aren't always.?

Effective? I have a cousin that got rpegnant and she was taking birth control. What are more effective ways?

I think that most of the problems are human error. If you take the pill at the wrong time in your cycle then you will be taking the sugar pills at the wrong time as well. I guess though that the body is also able to fight to reproduce, that being our basic chemical makeup. Condoms should be full proof barring any tearing of the condom.
