Depends on if you mean a canker sore in your mouth or on your lips/around the mouth.
On the outside, it is often caused by Herpes Simplex Virus 1, or HSV1 which is considered oral herpes or non-genital. Not to freak you out or anything because it is quite common and many folks have had cold sores.
Inside the mouth, however, is usually a different story. I have ended up with canker sores when I was sick or had injured my mouth while eating or brushing teeth. If you don't brush your teeth or clean your mouth out often, canker sores can result. I don't think I've ever had more than 2 and they were caused by biting the inside of my cheek so there were two spots that developed into them.
Anyhow, if they hurt-- get some over-the-counter numbing goo to put on them. It usually stings for a few seconds but then you feel OH-so much better. Also, rinse your mouth out more often to clear the sores of food and stuff.

There have been studies that suggested that this inflammatory disease is a result of abnormal immune response directed toward the oral membranes. Several viruses and bacteria have been investigated, but none has been found to be responsible. I've been lucky and have never suffered from a canker sore.
