If your original records were shreaded, then there is no way to get a copy. What you can have done is a titer, to see where you stand and if the vaccinations are still effective.

well i have to say this is funny to find out this happens to other folks, my files were shredded to and when i went to apply for my CNA i had to have a shot record and i have no records, so i went down to the health department and got my shots, i had to start from scratch, i think it is a total of 6-8 months worth
Have they got in on a database?
I went for a new patient health check at my surgery and as well as having my original file they also have all my notes on the computer.
Find all the shredded paper and stick it all back togther like a massive jigsaw?? Or maybe they have your records on computer. If not I am afraid it is down to hard thinking and use of your memory .. or start all over
