Where can I get a copy of my immunization record?

Question: I need proof my immunizations are current to apply to graduate school. I don't think my pediatrician is even practicing anymore, I'm 32. I have a hep B titre and am getting TB test, but where can I get proof of my MMR, polio, etc.? Can titres be drawn for these as well?

My daughter just did this for her school. She called the health clinic and asked, they just mailed it. Im not sure though how long they keep them. She's 18. But give them a call its worth a shot, they may be able to tell you where you can get them if they dont have them.
