A lateral release is done for the treatment of a patellar-femoral disorder where the knee cap tends to tracks toward the outside of the knee. After prolonged lateral tracking, the retinculum (which is a group of fiberous tissues) tends to get "tight," thereby further facilitating the problem.

Physical therapy after a retinacular release usually involves three basic prinicples. First is to reduce pain and swelling from the trauma of the surgery. This may include cold packs and/or the use of modalities such as interferential current. The second is to restore the range of motion and prevent scar tissue. This usually involves some type of streching. The third is to promote nomal mechanics of patellar tracking. This may involve stregthening excercises of quads...particularly of the vastus medialis obliques, but may also include addressing any anatomical anomalies such as foot problems or addressing lack of flexibility in the hip...whatever was driving the abnormal tracking of the patella.
