There are far too many to bring it down to just one, there are different types of tampons that carry different problems, there are also various different areas where there are problems regarding tampons; health, politics, environmental, social, historical.

I would probably say the number one problem is that commercial tampon manufacturers keep their customers loyal by making them feel bad about their bodies and menstruation – i.e. you have to plug up your vagina with bleached synthetic materials that risk your health in order to deal with dirty, gross, smelly menses.
It's for this reason so many women are unwilling to learn about risks or alternatives, it's also for this reason we see most of the problems with tampons.
For example dioxin in the 1990's the only reason for this going on was because tampon manufacturers wanted to make their products whiter-than-white to play to the idea of menstruation being dirty, as a result they risked a lot of peoples health, not just menstruating women but men and children too.

What problem? They work great for bloody noses.
