I've asked this question a number of times and have never gotten an answer from anyone who had anything nice to say.
I finally made my own.
I too am incontinent and wear diapers 24 / 7 due to a medical condition. I searched the web literally for days and could not find a pattern for either diapers or plastic pants.
I even emailed one of the baby diaper makers and did get a very pleasant and understanding response. She stated that she had tried to make them before but that they were too much trouble and she gave it up. I guess it has to do with the size of them.
The only problem with buying ready made diapers is the cost, they are so expensive to purchase and disposables have a long term expense that makes the impracticle not to mention that they are uncomfortable as all get out.
What I finally did was I took a disposable that fit well and layed it out, cut the elastic so it would lay flat. I made a pattern from it out of some card stock (manilla folders) that I could reuse.
I bought the material I needed and cut two pieces to match my pattern (make the wings 6" or more longer than the disposable).
Then I cut 6 pieces the length of the diaper and the width of the crotch as soakers. Take the 6 pieces and sew them together around the edges and once down the middle.
Lay this soaker on the wrong side of one of the panels and stitch it to the panel. Sew the two panels together wrong side out all but one end. When finished turn everything inside out and sew the open end closed. Put elastic at the legs and waist if you want to give a better fit.
I used velcro on the wings to hold the diaper closed when I wear it. No pins required unless you want that extra security.
I used the same pattern for the plastic pants except the wings are shorter than the disposable because you don't need any overlap.
Sew the ends of the wings together wrong side out using a straight stitch the fold the edge over and flat stitch the seam again with a zig-zag stitch.
Cut some 1/2 inch elastic to length that will be comfortable around your waist and legs. Two pieces for each leg and two for the waist. Sew two pieces together the length of the elastic to form a place to put the plastic between the two pieces of elastic. Sew the ends together. Put the plastic between the two pieces of elastic, stretch the elastic and sew through all three layers. Do this for both legs and waist.
I've made several pair of pants and half a dozen diapers. Try each one before making another in order to adjust fit. Each one you make gets better than the one before.

Why don't you buy SUMO outfits? You could also just wear a sheet neatly draped around your nether regions--it's not like it's going to matter if they are fashionable anyway, you are wearing a diaper after all. And, I think this is why they make disposable diapers? Depends? Why not just buy them from the store like everybody else, or do you like having your caretakers wash the poop and pee out of your soiled garments? Uggg.
