There's the presence of harmful microorganisms which gain entry to your ear or maybe in your respiratory system. As our body's defense against this bacteria, our white blood cells, monocytes, lymphocytes as our second line of defense tries to kill and engulf those pathogens by secreting lysozymes ( enzymes which are toxic to these pathogens that cause the infection).
As these monocytes engulf those bacteria and secreting lysozymes, they eventually die which creates the pus in the site of infection.
The mononuclear phagocytic system of our body will then stimulate the production of monocytes and bring them to the area of infection to clear up the area for wound healing.
The mess or trash created as they clean is the pus.
In your case, the pus and the blood is situated at the back of your eardrum. These blood and pus are the exudates formed after infection. They may contain the bacteria that were killed by our White blood cells and the dead WBC during the fight of these WBC with those pathogens.
If our respiratory system will be in severe infection. There is a great possibility that the bacteria will spread upto the ears. There's a connection between our ears with our oral cavity as well as the upper respiratory passages. These tube is called Eustachian Tube.
There's also a possibility that the infection in your ears will spread to your respiratory system through the Eustachian tube.
Treat this immediately to avoid hearing loss. Your middle ear was unable to drain the pus that accumulate in your inner ear.

Most ear infections are caused when the eustacian tube is blocked due to congestion from a cold or flu. The middle ear cannot drain to the throat properly and gets infected. Damage to the eardrum from using a q-tip or other pointed object to clean the ear canal is another cause.
Such ear infections should be treated ASAP because they can cause hearing loss.
