Answers:
1st off why are you still using a Doctor you feel is under performing? There are lots of medical graduates who were c & d students. Definitely find a primary care physician who you feel comfortable with and who you feel is looking after your best interests.

You might want to take a look at how your insurance handles this issue payment-wise. They might not fully cover the specialist if your not referred. & it seems that most docters work in tandem with the insurance company to keep patients away from higher priced treatment. & ask the oncologist how they handle billing. If neccessary force your docter for a referral (though, you should probably research oncologists in your area 1st & tell your dr who you want to be reffered to.). No docter wants to be accused of withholding treatment options.
Go to your doctor, say you aren't happy with the treatment you are getting and insist on a 2nd opinion FROM A SPECIALIST
