What do I need to do before I go in for physical examination?

I'm going or a physical examination for the first time, and I'm getting conflicting information on how to prep for the examination. What do I need to do before I go in?

Take any family medical history with you, shot records, insurance info, permission to be treated if your under 18.

edit: its more like you need to take it in the head, just know your stuff. insurance info you'll need physically though.

unless the doctor's office has told you specificly to to do something, nothing is what you need to do other than bringing yourself and any insurance cards,,,,,get there about 15 minutes early to fill out paperwork.
