The Chiro will have you fill out a medical history form a lot like going to a regular Dr. They will also ask you if you have ever been to a Chiro before and are usually willing to explain EXACTLY what they are going to do. They even have a spine model there and can explain what they feel is going on in your specific situation. Some Chiro's take xrays automatically, some dont.
If you fee uncomfortable with the way the chiro is going to adjust you, let them know. They have multiple ways they can do each adjustment. For example, my Chiro cant adjust my neck the "usual" way due to arthritis. I simply cannot tolerate it. So he uses what I call his "thumper" which is a tool he uses to "thump" the vertibrea back into position instead of the more common manual way.
If youre going because of a chronic pain issue, you might consider taking an Advil or some such so that the Chiro can adjust you with less discomfort, but in general, Chiro's are very gentle and considerate of your comfort level. deal with any issues before they can occure.
However, I have been to a couple of Chiro's in my day who were "turn you into a pretzel and jump on you" kind of adjusters. I learned to stay away from them. Chiropractors' styles are kind of like diets. They all work, but not for everyone. =)
You might also consider massage to go along with your Chiro visit, especially if you dealing with a chronic pain issue. Its what I have to do and I find the adjustments last longer if I also visit a massage therapist.
Also, remember to ice the area when you get home because moving your vertibrea back where it needs to be can be a little rough on the muscels that have gotten used to those areas being "out". Ice will help

Not painful at all usually. He/she will probably do an adjustment and if you are coming in because of pain you will feel better for a little while and then feel the need to go back to the ciropractor :)
