just the ear pressure changing that might cause discomfort. if you can-try not to feed him/her until you get on the plane and are in the air for about 15 minutes-that way the sucking on the bottle will help with the ears popping. plus with a full belly-he/she will probably sleep!!

you can also try a pacifier-the sucking will help the ears.

i flew with my daughter when she was 3 months old-the pacifier worked great-she went right to sleep

and as far as germs go-there are no more germs on an airplane than there are in the air he/she breathes normally so no worries there.
