If I understand your question. I can only speak for Europe and USA. Everyone will receive emergency care gratis. Anything beyond that requires proof of insurance. For most countries in Europe, that means you must have the legal right to live and work there. In USA, you need a health care plan that you pay into via your employer or on your own, or from a MSA (medical savings account) or personal funds; Switzerland's system is similar. Some states in USA have free or very low cost health insurance for those who are there legally and can prove low income.

The bigger question, if I may guess, is why don't the world's poorest have access to medical care? It's a good question, and the answer, I believe, is complex. The roots have to do with distribution and management of resources by a country--things which are influenced by it's philosophy toward medical care, how much money (tax) or resources are available, level of corruption, level of competence, level of ignorance, or a combination of these factors.

Mercy Ships, a ministry that visits ports of some of the world's poorest countries, offer medical treatment for free. But as good as their work is, the need is probably 100 times greater than their current resources.
