'Direct care' could vary from personal care as in helping to wash or shower or get dressed or cook a meal. This is a type of activity which a person will need in order to be able to live in a satisfactory lifestyle.
'Indirect care' can be anything else that needs doing, posting a letter paying a bill, buying a present visiting a friend. All these thing are nice things but are not life threatening if they are not done.
