I'm assuming this is the first time you have done this type of work. So let me give you some professional advice.
You should start by talking to the patient. He may be old, and he may have memory/cognition loss but that does not mean he can't tell you what he wants or how to do something. Get additional information from the family or clarification to anything you did not understand from him.
If possible, meet with the social worker, and his main nurse from the nursing home (you have to get the patients/families permission and possibly a release of information to accomplish this). They will be able to give you better answers to his care and abilities than the family can. It probably wouldnt hurt for you to talk to therapy (if provided in the home or by an outside agency) and ask them what areas you will need to physically help him with. They can also give you some pointers on how to properly transfer, walk, lift, etc... to reduce the possibility of you injuring him.
Above all else, make you you remember he is a human being that needs your help and he is the reason you have been hired. He should never be considered a "bother."
If he likes you, you like him, and you perform your job duties well... this could turn out to be a very rewarding experience for you.
If your able to get a copy of his latest careplan from the nursing home, that would help a lot.
