It's very difficult to answer a question like that online. Age and level of activity is a huge factor. You could have osteoarthritis, rheumatoid arthritis, bone cancer, torn meniscus, torn ACL or other ligaments, gout, or plain join inflammation from using your knees too much. See a doctor.

You could have what they used to call, "Water on the knee"; it's a build-up of fluid following injury. Or, you could've torn something in there.. then again. you may have injured the disc in your knee.. no way of telling without proper scans or, at least, xrays!
A year is a long time. If you are not better by now you probably have a meniscal injury which are common. These require a simple procedure to repair. They numb up your knee and but a little scope inside and repair it. A few days later you are as good as new! Good Luck!
