It could be myofacial pain syndrome (MPS) I used to have that and I had a PT (physical therapist) friend locate the pressure point to have it 'released'.
Tention and stress. Get a massage or strech.
It could be tension - if you're worried or upset about something, this is what usually happens.

Other than that, you may have pulled a muscle doing something - hard work or play can do that, unfortunately.

Try a heating pad for about 20 - 25 mins - not too hot and that should loosen the knots. If that doesn't do it, I hope you have someone that can give you a shoulder massage.
