Your sister has a blood blister. It's best to not pop it in any way because the skin is currently acting as a perfect bandaid. However, if your sister feels that it is too unsightly to leave and doesn't want to put a bandaid over it, you could use a sterile needle to puncture it very gently and just as gently squeeze out the blood. (Follow the above poster's directions about "lancing" it if you are going to pop it, just make sure to let the alcohol dry before poke it.)
But, the best thing to do is put a bandaid over it and leave it be.

Lance it. It would be best to take her to the doctor, but it can be lanced at home with a sewing needle. Wipe the bubble with alcohol (to prevent blood infection), burn the tip of the needle and carefully and gently prick the bubble. When the blood starts coming out of the bubble, apply gentle pressure to help the rest of the blood out of the bubble.
Go to a profesional don't listen to these people. Go see a local doctor or something.
Please dont pop the blister as this will cause it to be infected. You must treat the blood blister as you would a normal blister. Dont cover it with a bandain and dont pop it. It will eventually heal up and disappear, they usually take longer to heal than a regular blister so good luck, if in doubt and you are worried about it, maybe get a close friend/adult or a doc to check it out.
