Question:
My mentally challenged brother (age 25) has a fear of riding in cars other than the ones his immediate family drives and taxis. While on vacation, he would hesitate getting in taxis and when we were to enter the taxi, he would vomit. He knows this is an issue and gives us an apologetic smile. However once he vomits, he can then get in the taxi and is fine. We try to prepare him for the ride by explaining that we will soon enter a taxi and need him to be ready. There are times when he knows the family member but still has issues with their car. This also happens with some people's homes (again those that he has not been to). This does not happen when we take rental car shuttle buses, planes and taxis in Manhattan.

I would say go and consult the GP about a psychologist! This is a psychological problem and nothing to do with his IQ etc... Could happen to anyone! I know people who are far from mentally challenged but still have phobias and irrational fears/reactions to things these are to do with mind state and are not as uncommon as you think!
do you live in manhattan ? maybe he is only comfortable in his usual surroundings and nervous/anxious in strange or new places he isnt used to
