Does Tylenol's effect wear off in a month or so?

I have been taking it for maybe 5 or 6 weeks, about 6 pills (500mg) a day. It doesnt seem to work like it did, for my achy back and other things. I prefer it over ibuprofen because I can take it on an empty stomach. And ibroprofen comes with so many warnings about dosage.
