Yes it will straighten your nose. Usually an ENT surgeon does it but it can be done by a plastic surgeon. Only the surgeon will be able to tell you if they will shape it at the same time.
Septoplasty will in fact straighten your crooked nose. I usually hear that these procedures are done by plastic surgeons, but I suppose an ENT could do it as well if they are certified in the surgical aspects.
As for suggestions on how to shape your nose, I would discuss that with your surgeon or ENT - whoever is doing the procedure. They see stuff like this all of the time and will be able to tell you what shape will work best for you.

septo's can be done by plastic surgeons or ent's. I recomend that you allow the doc to decide if any cosmetics are needed because you will be amazed how well you will beable to breath out of both nostrils and it will straighten your nose a quite a bit.
You need a septorhinoplasty, which is rhinoplasty and septoplasty performed at the same time.
