Yes it could break, but on the other hand it depends on the fall. Like the amount of impact and how the leg was twisted ect.
If his leg is swollen tell him to put ice of it, NOT on his skin, due a towel or zip lock bag. Putting Ice on the skin will case frostbite so I have been told and cause the skin to turn red.
If his leg is still swollen or turning I think is purple and he can't walk on it due to sharp stabbing pains, then yes it could be broken.
If I was him I would get it checked out ASAP.

Not only could it get broken,it could have fractured. I hope he got medical attention immediatly because a bone fragment could cause even more damage to surrounding blood vessels and result in internal bleeding.
