No. Sometimes, they might remove the entire nail (it happened to an ex-boyfriend and one of my cousin's friends), but usually, they remove the section that's infected/ingrown and put solution on it to kill the root. However, there's a 5% chance it will come back, as some people are immune to the medicine. That's what the podiatrist told me when I had mine done.
The surgery is done in office, and the doctor will give him about 2-3 shots in the area. That hurts worse than the surgery! I have had shots and blood tests many times, so needles don't bother me, but the toe is sensitive. I figured I'd better warn him. The doctor wraps the toe in the bandage. I was able to wear shoes after two days. After a day or two, you can wear a regular band-aid, but if he can wear sandals at work or school, I suggest wearing them for several days after. Usually, Advil will kill the pain if you take it on schedule, though some podiatrists will prescribe pain pills. If he's not on a prescription pain pill, he can go to school or work. I worked as a receptionist at the time I had my last toe done, and went back to work the next day. I just wore dress sandals and didn't take any pain killers other than Advil.
The only way he would have to worry about losing the toe would be if it was extremely infected and went on for so long that no one without an extremely high pain tolerance could handle AND if he had severe nerve damage and/or diabetes and nerve damage brought on by that.

no, they do have a surgery to fix it though! i had to have surgery....i was scared too
I had a really bad ingrown toenail, REALLY bad for a very long time. They just permanently removed the toenail. Its kind of gross, but it looks a lot better now. I dont think they would remove his toe, but he should go to the doctors and get it treated, it can/and probably will get worse.
